Solution to this is as follows
- Navigate to Legacy system administration >>Manage Person.
- Search for that patient using either name or identifier
- Click on the patient you have searched.
- Click on the patient dashboard link. In the dashboard check if there are two programs of the same type and if all still active
- Select the most recent and delete it with reason duplicate.
