Hello @RODGERS, that form was retired, its only available for reference.
If you want to enter data about TB, you use the DS (drug-susceptible) TB form to enroll the patient and then use the DS TB follow-up form to enter follow-up data.
If you want to enter 2nd line data, use the DR (drug-resistant) TB form for enrollment and the DR TB follow-up form to enter the follow-up encounters.
Please note that DR forms can only be accessed after you have completed the DS episode.
In case you get challenges in saving the follow-up data, kindly ensure that you are version 3.4.1 and above
